HealthWISE is seeking a dedicated Aboriginal Health Practitioner to join our team in Moree. This role is your opportunity to improve health outcomes for Aboriginal and Torres Strait Islander communities by delivering culturally sensitive care and contributing to an innovative multidisciplinary approach to managing diabetes.

What You’ll Do:
- Coordinate and deliver care for Aboriginal clients referred to our multidisciplinary team (MDT) model.
- Facilitate MDT clinics, group education, and healthy lifestyle sessions.
- Collaborate with GPs, allied health providers, and local health organisations to deliver integrated care.
- Promote culturally appropriate health practices and empower communities to make healthier choices.
What You’ll Bring:
- Experience or understanding of Aboriginal health issues and culturally appropriate practices.
- Strong communication and teamwork skills, with the ability to work autonomously and collaboratively.
- A passion for delivering quality, person-centred care and improving community health outcomes.
Desirable Skills:
- Hold Certificate II/III in Aboriginal and Torres Strait Islander Primary Health Care, or willingness to work towards same.
- Experience working with diabetes care or in rural/remote health care settings.
- A commitment to professional growth and community-focused health initiatives.
What You’ll Need:
- Current Driver’s Licence.
- National Police Check and Working with Children Check/Blue Card.
- Compliance with vaccination and safety policies.
