Indigenous Mental Health Care Coordinator

Full-time | Fixed Term Contract | Identified Position

Are you passionate about supporting Aboriginal and Torres Strait Islander communities? Join HealthWISE as an Indigenous Mental Health Care Coordinator, working to improve mental health outcomes through culturally safe and coordinated care.

Based in Armidale, Tamworth or Glen Innes, this role involves outreach travel to nearby communities and collaborating with GPs, allied health providers and community services to deliver person-centred mental health support.

About the Role

  • Provide culturally responsive care coordination for people with mild to complex mental health needs
  • Promote cultural awareness and help build stronger referral pathways across services
  • Support GPs and other providers in delivering early intervention and preventative care
  • Facilitate access to Telehealth and local services
  • Contribute to health promotion and mental health education strategies
  • Regular travel (approx. 2 days per week) and overnight stays as needed

About You

  • Identify as Aboriginal and/or Torres Strait Islander
  • Certificate IV in Mental Health (or equivalent)
  • Experience in mental health care coordination and client-centred support
  • Strong communication, cultural knowledge and community engagement skills
  • Ability to travel regularly across the HealthWISE NSW region
  • Skilled in using Microsoft Office and health information systems
