Are you passionate about providing culturally appropriate care and supporting better health outcomes for Aboriginal and Torres Strait Islander communities? HealthWISE is seeking a dedicated Aboriginal Health Practitioner to join our Moree team and be part of an innovative, multidisciplinary approach to managing diabetes in our region.

In this role, you’ll work collaboratively with a dynamic team to deliver culturally sensitive, person-centred care for Aboriginal clients living with diabetes. From facilitating group education sessions to coordinating holistic care plans, you’ll play a critical role in improving health outcomes and reducing avoidable hospitalisations.

What You’ll Do:

  • Facilitate fortnightly Multidisciplinary Team (MDT) clinics and group education sessions.
  • Collaborate with GPs, allied health professionals, and community organisations to ensure seamless care.
  • Conduct health assessments and develop care plans tailored to individual needs.
  • Support clients in achieving their health goals through culturally appropriate care and health education.
  • Utilise digital tools and technologies to enhance client engagement and service accessibility.
  • Act as an advocate for Aboriginal health and wellbeing, empowering communities to make healthier choices.

What You’ll Bring:

  • Strong understanding of issues affecting Aboriginal and Torres Strait Islander communities and their health.
  • Proven ability to work autonomously and as part of a multidisciplinary team.
  • Excellent communication skills and the ability to engage with diverse stakeholders.
  • A commitment to providing culturally appropriate, person-centred care.
  • Willingness to travel and work flexible hours when required.

Desirable Skills:

  • Experience working in diabetes care or chronic disease management.
  • Previous experience in rural or remote healthcare settings.
  • Ability to adapt and thrive in a dynamic and evolving healthcare environment.

What You’ll Need:

  • Registration as a health professional (AHPRA or recognised peak body).
  • Current Driver’s Licence.
  • National Police Check and Working with Children Check/Blue Card.
  • Compliance with organisational vaccination policies.
