Fixed term contract
The ECS Connector has the primary responsibility of connecting with local Elders, older Aboriginal and Torres Strait Islander people, and their families to raise awareness of their aged care entitlements. The overarching function of the ECS Connector is to ensure that support for clients is organised, sequenced, and supported to enable effective access to appropriate aged care services both internal and external to the ACCHO.
Knowledge, Skills, and Capabilities
• Experience working with Aboriginal and Torres Strait Islander people.
• Excellent communication and interpersonal skills, with the ability to build trust with the community.
• Understanding of person-centered care and trauma-informed practice.
• Ability to assist clients in accessing services and simplifying service complexities.
• Sound understanding of My Aged Care and operational guidelines.
• Good record management, data collection, and reporting capabilities.
• Strong community connections and networks across aged care providers, mainstream supports, and community sectors.
• Ability to work autonomously with high initiative and problem-solving skills.
• Experience working in a team environment that is open, transparent, and focused on person-centered care.
Qualifications & Experience
• No formal qualification is required; however, qualifications in a health or care- related field and Current First Aid are well-regarded.
• Demonstrated experience in a health or care sector role working with older Aboriginal and Torres Strait Islander people.
